    ​In order to obtain quality radiographs and to perform a through dental exam and any other exams that will require the horse to remain still, the horse may need to be sedated.  By entering your name in the signature box below and submitting this form, you are giving approval to sedate the horse indicated above if deemed necessary by the examining veterinarian in order to assure safe, thorough and effective diagnostics and examinations.
